Instructions for patients on oral anticoagulants

 

  1. Memorize the name and dose of your tablet.
  2. Take the tablet at the same time everyday e.g. at 6 PM every day.
  3. Keep stock of at least one week handy.
  4. If you have forgotten to take the tablet on a particular day then, DO NOT compensate by taking double dose the next day.
  5. PT /INR blood test should be done once a week while on the tablet. The INR should be maintained between a range on 2 and 3.5. In case the INR exceeds this range then withhold the dose for one to two days and inform your doctor. You will be guided on when to restart the medication and at how much lower dose than the earlier daily dosage and when to repeat the INR again.
  6. When you do the blood test and inform the report, you must also inform your Doctor if you have missed any dose.
  7. In case you have forgotten to take the dose of the tablet for more than one day then do the INR blood test only after you have taken the tablet regularly for one week.
  8. In case of heavy menstrual bleeding withhold the dose temporarily and restart after the bleeding has stopped. Inform your Doctor the number of days the Tablet was stopped. Recheck the INR one week after restarting the tablet.
  9. Avoid injuries: In case you bleed then apply compression over the bleeding area for half an hour and check that the bleeding has stopped completely. If not, then contact your doctor immediately.
  10. Do not self medicate for any other illness while on anticoagulants. Always check with your doctor before taking any new medicine while on anticoagulants.
  11. Pregnancy must be avoided while on these tablets.
  12. Avoid using oral contraceptives while taking oral anticoagulants.
  13. Avoid long distance travel while on these medications. If travel is unavoidable please use full length stockings. Get up and walk around for a few minutes after every half hour to promote blood circulation.
  14. Exercises of lower limbs and feet should be undertaken at regular Intervals. Do not sit for prolonged intervals in the same position or with legs in cross-legged position or with legs dangling for long periods.
  15. In case stockings have been advised to you the correct method is to wear them immediately after a bath and to remove them only at night before sleeping.
  16. Do not use liniments or gels while on these medications. Hot fomentation is also to be avoided while on these drugs.
  17. Avoid foods rich in Vitamin K such as Basil, Thyme, Sage (dried), Parsley, Amaranth leaves, Kale, Mustard greens, Spinach, Basil, fresh Tulsi, Beet greens, Coriander, Turnip greens, Lettuce, Broccoli.
